AGREEMENT & WAIVER FOR WELLNESS PROGRAM
AGREEMENT TO PARTICIPATE: I have read and understand that my participation in the wellness program would requires me to provide all the necessary information for health practitioners to evaluate and assess my health status for the purpose of improving my well-being. I shall commit to my best effort to ensure that I will achieve the intended outcome from the program.
ASSUMPTION OF RISKS: In this wellness program, the health practitioner may recommend some physical activities to improve the health status of the participants. However, physical activities by its very nature, carries with it certain dangers and risks that cannot be eliminated regardless of the great care taken to prevent or minimize harm. I, hereby assert that participation in the program is voluntary and knowingly assume all the related risks.
PRIVACY POLICY AND CONSENT: I have read and understand the privacy policy stated on https://teleme.co/privacy_policy and give consent related to use of the platform.
ACKNOWLEDGEMENT OF TERMS OF USE: I have read and understand the terms of use stated on https://teleme.co/terms. I agree to abide by these rules and standards for the uninterrupted operations and safety of all participants and health practitioners.
WAIVER: In consideration for Teleme accepting the enrollment of Participant in a program and/or permitting Participant access to or the use of the platform, Participant, as well as his/her heirs, next of kin, and personal representatives hereby waive any claims against and covenant not to sue or bring any action against Teleme, its affiliated companies, or any of their members, directors, officers, employees, volunteers, sponsors, independent contractors or agents, for any claim, demand, or lawsuit whatsoever, including, but not limited to, those that arise from or relate to Participant’s own acts, the acts of third persons, the effect of the condition of any property, equipment, or premises, or any acts of Teleme’s own negligence, or the negligence of any Teleme officer, employee, agent, or anyone else whose conduct may be attributed to Teleme.
INDEMNIFICATION: In further consideration for Teleme accepting the enrollment of Participant in a program and/or permitting Participant access to or the use of the Platform of Teleme, Participant, as well as their personal representatives or assigns, hereby contractually agree to defend and indemnify Teleme, its affiliated companies, or any of their members, directors, officers, employees, volunteers, sponsors, independent contractors, or agents, from any and all claims, demands, lawsuits, or damages, including related costs and attorney fees, brought by any other person or entity for any injuries or any damage to themselves, their property, or to Participant or his/her property, arising out of the use of any Teleme service or facility by Participant and/or his/her guests, relatives or family members.
